In order to request facility use within the Waterloo Community School District we have implemented a new process/procedure and fee schedules effective July 1, 2021.

To request use of our facilities, you must complete the Facility Use Request Form and provide a Certificate of Insurance Liability. Both forms must be submitted to Lorelei Tack either by e-mail, in person, or by mail for review and approval at least 10 days prior to your first requested date of use.
Required Forms Include
1. Facility Request Form and Agreement (See links below)
2. Certificate of Liability EXAMPLE CLICK HERE indicates the Waterloo Community Schools as the certificate holder and listed as “Additionally Insured”.
3. Copy of IRS letter indicating non-profit (501 (c) 3) status, if applicable
It is important to submit all completed forms at least 10 days prior to requested dates of use.
There are a few options to submit the forms.
IF you have Microsoft Word on your computer, you can fill out the form electronically by CLICKING HERE for the Facility Request Form which will open in your browser. You must then click to download and click to “enable editing”.
